An individual at least 18 years of age with a high school diploma or equivalent degree is determined to be qualified to provide the service.
The career ladder is designed to advance upward. It is not permissible to step down the ladder from any level.
QMHS
Application Requirements
MHRS Qualification
Letter from DHCS, County Authority, or designee agency verifying prior MHRS Qualification.
Official ID
Upload a copy of your official government ID, such as a driver’s license, passport ID, or State ID. This is a step in the application.
Mental Health Code of Conduct
Affirmation of CCAPP Credentialing Mental Health Code of Conduct.
Completed QMHS Application
Complete all steps in our online e-application system, called Certemy.
Educational Certificates
Copies of educational certificates (trainings, workshops, seminars, conferences, etc.)
Transcripts
Copy of transcript or proof of high school diploma or equivalent degree (previously primary source verified by your employer).
Fees
The QMHS application fee is $100 non-BHAP or $50 if a BHAP member. This fee is non-refundable.
